I am trying to help a 40 y/o primip with a term baby who lost 16 oz in
two weeks after birth.  There were a lot of issues to be corrected and
baby is now gaining and within 3 oz of birth weight in only five days.

The remaining issue is handling Mom's pain while breastfeeding.  Baby
appears to have a proper latch on - - lips flanged, tongue forward over
lower gum ridge, well up on the areola.  She has pain in the nipple and
breast throughout a nursing session, and relatching does no good.

When the nipple comes out, it is flattened.  Baby has a broad palate
with only a light arch, strong tongue action and strong suck.

Mother has a low milk supply, so she is pumping and adding formula
supplement with an SNS as she nurses.  The baby drains the SNS rapidly
as she nurses.  She will try Fenugreek this week to see if it increases
her supply and we have discussed Reglan if necessary.

Does anyone have any advice I can pass along to her to make her more
comfortable while breastfeeding?
