Lactnetters:  I am researching barriers to breastfeeding exclusively for six
months.  My goal is to publish my findings and draft a questionnaire for
LC's and other hcp's to use to help them identify these barriers in mothers,
in order to help mothers overcome them *before* they negatively affect
breastfeeding.  
 
Also, I am interested in the possibility of doing some sort of study in
which mothers who have been identified in third trimester as having a number
of these barriers are matched with an IBCLC for long-term, consistent
lactation management (care).  Control would be mothers with similar
barriers, but no consistent care.  If you would like to help with such a
study or know of resources that would be beneficial to it, please let me
know.  I am not affiliated with a university nor a hospital and may need
partners who are.
 
Ultimately, the idea is to raise long-term (exclusive to six months, and
continued into the 2nd year) breastfeeding rates in my Province/Nation
(Canada), but naturally this information could be useful in many other
regions and countries also.
 
Below is a list of barriers that I myself have brainstormed using my own 10
years of bf counselling; if you have any others to add, please let me know.
Also, if anyone here is aware of any published studies which clearly
demonstrate one or more of these barriers and methods for overcoming same, I
would be very grateful for the link.  As well, if you know of any articles
or other publications dealing with this issue, I would appreciate it if you
could share such with me.

BARRIERS to EXCLUSIVE BREASTFEEDING 

for SIX MONTHS

 

 

£      INTENT:  Lack of intent to breastfeed.  Lack of intent to breastfeed
for longer than a pre-conceived idea of what is acceptable.

 

£      CONFIDENCE:  Minimal confidence with the ability to breastfeed.
Minimal confidence in overcoming challenges during breastfeeding.

 

£      KNOWLEDGE:  Lack of knowledge and understanding regarding the meaning
and significance of exclusive breastfeeding.

 

£      INFORMATION OVERLOAD:  Too much information from too many different
health care professionals, especially with lack of cooperation between
professionals.

 

£      INTERVENTION:  Labour, birth, & post-natal practices that undermine
breastfeeding.  Ineffective counseling from health care professionals not
qualified in Lactation Management.  Inappropriate use of lactation devices
such as pumps & shields.

 

£      FEAR:  Negative anticipation of pain or discomfort.

 

£      SUPPORT:  Low interest in mother-to-mother support services in the
community.

 

£      RESOURCES:  Few resources for mother-to-mother support in the
community.

 

£      SIGNIFICANT OTHER:  Ambivalence and/or hostility from the significant
other.

 

£      NEGATIVE EXPERIENCE:  Prior unsuccessful or unsatisfying experience.

 

£      BODY IMAGE:  Maternal struggle with the dual role of the breast:
Nourishment and Sexual.  Resistance to the physical demands of
breastfeeding.

 

£      NIP:  Concerns regarding nursing in public.

 

£      SEPARATION:  Mother and baby separation due to return to work,
studies, volunteering, personal interests, etc.

 

£      ABUSE:  Past or present sexual, physical or emotional abuse.

 

£      FERTILITY:  Lack of knowledge about the LAM method of birth control.
Fear of becoming pregnant.   Desire to conceive quickly.
