My daughter who weighed 5lbs 14.5 oz when she was born dropped to 5 lbs 4 oz by day 3 despite ad lib breastfeeding by a seasoned LLL Leader mom.  This was back in 1982 and no one freaked out about the drop in weight.  On day 4 she had gained an ounce and by day 6 had gained an additional 2 ounces.  From that point on she did just fine.  I suspect that part of the problem may have to do with different scales, and weighing the baby in inconsistent patterns (for example a baby who got weighed right after a large meconium stool was passed is going to weigh significantly less than he would have a half an hour before).  At this point I would pay more attention to his output than any one weight on a scale.  Since he appears to be gaining, and is producing appropriate amounts of poop, I'd continue to nurse frequently following feeding cues, and waking him if he doesn't wake himself by 3 hours and then check in with another doctor visit next week.  The  sometimes on and off behavior doesn't sound all that odd to me.  My granddaughter did the same thing and she gained like gangbusters.  My own daughter had a similar sometimes nurse for 45 minutes sometimes only for 5-10 pattern at that age.

Keep offering soothing words over the phone and I'll bet you'll find he's gained like crazy when you finally get there.  It's hard to be grandma a phone call away instead of grandma on the scene.  Believe me I know.  But, it honestly sounds like things are going pretty normally at this point.
