I just talked to the daughter of a friend of mine who lives in Europe
and is going to be coming back to the U.S. for a visit next week. She
reports that her 4 month old baby has been difficult to nurse since a
week or so after birth. Baby born term, normal weight and nursed well
after birth. Mom had sore nipples at the beginning which lasted for 5
weeks  . Mom states she had overactive letdown and baby coughed and spit
up some, but that resolved after another month.  The difficulty she is
having now is: baby is very particular where she nurses, can nurse only
at home, not even in other quiet environments; she is very fussy when
going to latch, takes a while to get her on then she nurses for 4-5
minutes at most feeds, pulls off and starts crying and will not relatch.
Mom settles her by holding walking and giving her a pacifier, and then
she is happy. She has "normal breastfeeding stools, large yellow seedy,
every day or two. She has always had a small reddened area around her
rectum. Baby has a lot of gas and Mom has quit dairy which she thinks
has slightly improved the baby's gas. The baby has not had any bottles
through this time, and has gained an adequate amount of weight, although
is beginning to slow down each month. She nurses every 2 hours on
demand, with a longer stretch at night. No S/S of yeast. Mom has been
unable to pump with a good pump, can do some hand expression, leaks on
one side while she feeds on the other. She does a little hand expression
before the baby lathes. Baby no longer coughs and Mom states she
swallows with each suck, and when she sucks two or three times before
swallowing, that is when she starts to get fussy. She does not like to
be held tight, nurses only in the football hold (Mom has large breasts)
does not like slings and will only face out when in her carrier.  Has
been checked by a Pediatrician, report normal palatte, tongue. Mom will
try elimination diet of all dairy and soy, breast compression, more skin
to skin. Is very worried about feeding her in aiports and the airplane
due to her being picky about her nursing environment. Any other ideas or
what should I look for when evaluating them next week when I see her?
