Im sorry, I think I didnt identify myself on my last email...(writting very late at night when my computer is free from teenagers!!!).
   
  Regarding follow up to mothers and babies discharged with a nipple shield, I do see them from 3 to 7 days after discharge.  Not all mothers leave with the same instructions, since every baby and every mother are individuals.  I am aware that sometimes what we do see in the nipple shield is saliva or formula (if we are supplementing with the peridontal syringe at the breast).  When I make recommendations to the mother, I do take in consideration the baby's stool output, and discharge weight.  Many times I do have to recommend to the mother to supplement.  
   
  My point is that the nipple shield is an excellent breastfeeding aid.  Of course, we, as lactation counsultants, LLLL, peer counselors, etc. have to be aware that we do not jump to use BF aids at the first chance.  We are encouraged to treat bf using the least technollogy as possible (at least this was the "moto" when I became a LLLL and IBCLC).  I see more abuse of breastpump use this days than the nipple shield...but that's another story.
