Dear teachers,

I was asked to visit a mom who had recently given birth to her twin babies 1 week ago. They were 8months premature. Mom had been trying for the past 8 years to have a baby and they were conceived via IFV (hormonal issues at mom's end)

They were delivered in our one and only private hospital (also known to be not baby friendly, especially the midwives) I was surprised to learn that babies were fed with a special preemie formula for the first 3 days and they did not ask mom to express her colostrum until about the 3rd day when mom asked them shouldnt she be expressing to feed her babies? And their reply was "Oh yeah, if you want to, you can express else they are doing just well on formula"

Mom was adamant to breastfeed them and she did a couple of hand expressions and pumped some milk out using a breastpump. The babies are being fed every 2-3hours and so far Twin A is actively nursing (which makes it more easier for mom to bf) but Twin B has already developed a nipple preference as the midwives fed the formula using a bottle. It takes a little work, with hand compressions while baby is at the breast, but eventually the baby nurses. Mom is finding it quite tiresome and at times she is worried that her underlying health condition might not enable her to produce enough milk.

Mom had asked if her babies could also be on donated EBM? Her friend, who has a 6 month old baby has an excess of 100+bags of EBM stored in packs of 4oz. If there are no issues of a preemie taking EBM from a mother of an older baby, the mother would like to proceed with this.

Is there any protocols on sharing of EBMs especially with preemies? We do not have any Milk Bank here and sharing of EBMs is only well accepted within members of the same family due to religious aspects, hence the lack of info on this.
