You mention that mother has been lactose free, but what about milk protein free?? It's rare for a child this young to have a true intolerance to lactose anyway. That usually develops much later, often not until the late teens or early adulthood. Has the mother tried a milk elimination diet removing all milk sources from her diet for at least 2 weeks; ie. milk, icecream, creamed soups or dressings, sour cream, cream cheese, anything with whey or casein, cheeses, yogurts, etc.? This is more likely than lactose intolerance. Ask her about family history on both sides. Any aunts, uncles, cousins, etc. who just couldn't drink milk or who were told they were lactose intolerant? This is a red flag for inherited cow's milk protein sensitivity. See the information on this site about an elimination diet:
