We use a consent form for formula at our hospital, as well as consent for nursery care (separation), pacifiers, and nipple shields.  (All consents are on the same form)   The reason is continuity of care and liability.  We want all patients to receive accurate information, and having a consent allows patients to receive the same information regardless of who the nurse is.  Also, from a medical-legal perspective, we know the risks of not breastfeeding, and the hospital is at risk by not informing patients.  Once you have the legal team on board, it'll be really hard to remove the consent regardless of which docs sit in your peds-perinatal group.
