I've held off adding my 2 cents' worth, as others have contributed websites and mothers' groups that could provide you with more immediate help for your client.  
If you have access to an Australian journal, Breastfeeding Review, a case report I published there may be of interest.  It's about a mother with very little glandular tissue at all, who was able to keep her babies at breast for 2-4 years, even though most of their nutrition came from artificial baby milk.  She is a remarkable woman.  This mother didn't have tubular breasts, just proportionate breasts with limited glandular tissue and wide spacing.  The colour photos I submitted wouldn't reproduce in the article, unfortunately.  The photo of a Mum using a tube supplementer that the editor placed to accompany the article was of a woman who *did not* have insufficient glandular tissue.
