Hello,

A quick reintroduction as it has been awhile since I posted.  I am an IBCLC
at a small hospital in Pekin, IL. I see both inpatient and outpatient moms
and babes.

I have a mom with a 7 month old baby who has had one sided nipple pain for
the last month.  When I first saw her, she had a plugged duct on one breast
which came out easily and on the other nipple she had a small white patch
in the center of the nipple.  She described burning and shooting pains.
Thinking yeast, I suggested she try gentian violet which she did with no
change in her pain level and no change in the white patch.  Babe has never
had oral thrush, but has had some diaper rash. She went to her family
doctor who did a needle aspiration of the patch on her nipple.  She
obtained (according to mom) lots of blood and pus, which was cultured for
bacteria and was negative.

I saw mom again today and the nipple and breast pain continues.  She
describes it as sharp, shooting pains that radiate through her breast
and "come right out the patch on her nipple".  She also has a small lump at
12:00 at the edge of the areola.  She claims that the lump, which is about
the size of a pea, is always consistent in size.

Mom also states that she has had lots of sinus problems over the past four
years...I am trying to mention anything that might be relevant here...

I keep thinking yeast, but I wonder if I am missing something.  That
consistently sized lump bothers me.  Any ideas?
