As well as being the "right thing to do" in terms of promoting teamwork,
HCP reports are also a very strong marketing tool.  I send at least one
for every client I see and have been getting many clients referred by
their physicians or midwives.  One office manager told me their office
used to refer to another LC, but never heard anything, so they were not
sending her any more clients.

My forms are very simple, an assessment checklist and a space for a
summary paragraph.  I designed them myself in a word processor in a few
hours.  It takes about 15-20 minutes per report.  If my assessment turns
up red flags of a medical nature, I call the doctor, otherwise, I just
mail the form.
