Sorry I am few days late on response to this. I vote mastitis/chronic
mastitis which can be seen with oversupply. If I were to suggest a culture I
would do it similarly to a clean catch urine culture. That is, have the mom
clean her breasts and nipples of surface germs (phisohex wash, betadine, or
just soap and water), wash her hands, then hand express some milk out to
discard, then express the next milk in a sterile specimen jar. Send for
culture and sensitivity. This should tell you what to treat with.
