Hi, I am a bit sad to see peole slagging this study off.  The work was done 
in the 1990's for the original study, and when the first PROBIT paper 
appeared, there were enthusiastic welcomes here on lactnet -- I remember 
this, as I was a lot newer to critiquing research then and some of the 
comments really helped me to understand the importance of the paper. 

Over the years, this paper has helped to build a solid foundation of 
evidence for BFI -- which is almost now 'governement recommended' here in 
the UK --- Kramer evidence has been an important part of it.  Having 
conducted the study and being in a position to follow up -- this means that 
the study can go on helping us understand the implications of BFHI for women 
and babies.  Of course the study is not perfect, and there may be 
methodological weaknesess in the follow up (I haven't read it). 

Its like the Howie research, which was incredibly influential in the UK, and 
which now looks somewhat naive in its understading of exclusive 
breastfeeding -- if these researchershadn't broken the ground, our ideas 
which flourished to our current understandings would not have been able to 
grow.
