Hello, 

I am a (hopefully) soon to be IBCLC in Texas. Awaiting test results in just
23 days! In preparation I am working on forms for my future private
practice. I would like to hear any varying opinions on creating and using
care plans. Seems to be several different types of care plans and for those
in private practice I would like to hear what works best for you and why. I
have seen several examples with pre-written text and check boxes and some
with just a full blank page to be filled in per the visit. If anyone is
willing to share a copy of your care plan, I would be very grateful, you can
