Breastmilk jaundice occurs in infants who are gaining and thriving, and occurs when mature milk comes in. It can certainly last for weeks.
If a mother has an infant with breastmilk jaundice she will have subsequent infants who have the same.
The infant in question likely had other factors leading to early hyperbilirubinemia and then combined with breastmilk jaundice which kept it going for weeks.
Other factors contribute to early jaundice and each of her subsequent  infants could have some or all of these factors, so subsequent babies may not present the same as this one.
Factors that increase jaundice (bilirubin levels): prematurity or early term infant, bruising, NPO (nothing by mouth), delayed feeding, ineffective or infrequent feeding, ABO incompatibility, maternal diabetes.
I agree that a level of 23.5 on day 4 would indicate some or all of those early factors were at play.
