Deborah,
I've had two cases of large-nipple moms, both were able to breastfeed after
using their thumbs (or Dad's) to finger-feed or suck train.  One baby a 35 week
preemie, surprised me by learning to nurse this (to me) "very large" nipple
while still in the hospital.  The second took over six weeks of suck training
and finger feeding.  I instructed the mom to use no pacifiers and not let the
baby suck on his thumb, but rather have baby suck on her thumb and get used to
being comfortable with something large in the mouth.  The thumb was never to be
introduced so far that the baby reacted negatively towards it, rather slowly
allow baby to "suck it in." It took time and persistence, but she did fine.
To maintain milk supply one found the G--tle E------ns pump's flange large
enough not to irritate the nipple.  The other used the large insert for the
M-----a Pump.
