Jan writes of the benefits of universal home f/u within 48-72hrs.
Having recently left a VNA providing just that, I agree wholeheartedly.
This time frame presents an oppurtunity to coax the poorly supported, reluctant
nurser back to the breast or provide that extra little encouragement to the
formerly highly effective dyad now struggling with a full breast!   It is also
the perfect setting to support parenting skills.
My current hospital does provide telephone calls to ALL within 48hrs of D/C
(9000+deliveries/yr) via a service we call "warmline". Patients are visited by
the warmline attendees p/t d/c , given a magnet with the phone # with the
option to call whenever.   IF the wl nurse detects a breastfdng issue, she
schedules them for a f/u visit in our brfdng outpt clinic.
Jan also writes about f/u issues relevant to the almost term baby.   Nancy
Wight wrote a truly wonderful piece for the May issue of Pedi. Annals addressing
these.   It has always seemed ironic to me that in the hospital setting and
given the same 35 weeker,my LC job often involves
"lobbying" the NICU staff for less invasive, more high touch care vs. the
nursery/ postpartum staff for closer observation and quicker intervention!
