I  have that full-breast/MER sensation too, especially when I am worried
about a little one.  Just worked with a little guy who did not seem to
have any idea what to do at the breast (and he was born at home and
unmedicated, and never seperated from mom), and was very hungry on day 2
having never latched.  I saw him several days in a row, got mom feeding
him by several different methods, and came home from the last visit
feeling very full and engorged!  Today I heard he is breastfeeding well,
softening moms breasts at each feeding.  Whew.
