I am a continuous lurker, and have occasionally posted a question or two.
I am the LC in a busy, level 3 NICU.
I have a Mom with 27 week twins, has been pumping her heart out and has a
ton of frozen milk.  She has complained of chronic "uncomfortable" nipples
but has also been pumping for 30-45 minutes per session (on minimum
suction)  with a hospital grade, double electric pump. No cracks, bleeding
or oozing, but the nipples are red. I have discussed limiting the sessions
to 15-20 minutes, we reviewed air drying, lanolin use, no soap, then we
tried mild soap and water.  We did not use breast shells to promote drying
because she also has a history of plugged ducts (that is another story).
Anyways, her nipples started becoming more sore with pains here and there.
More than tingling, yet not stabbing pains.  We thought initially that it
was when she was 'full', because as soon as she pumped, the pain would go
away.  The pain has been increasing so her OB/GYN has decided to treat her
for yeast with Diflucan and Nystatin.  We have also started the babes on
prophylaxis oral nystatin since they are going to breast and also
receiveing bottles of EBM.(They are now approx. 35 weeks)
My question is - should we feed the frozen milk to the babies since we
don't know when the yeast may have set in, or should she discard it.  It
would be  a shame to discard all that milk.  Then if we don't discard it,
and she uses it in a couple of months, are we now risking thrush to mouth
and nipples again....
Is it safe to feed premies milk from mothers that have yeast infections on
their breasts or should it normally be discarded?
