Annie's client has a 4 m baby who 'cries in pain while nursing at the
breast. On the bottle is
uncomfortable, but better and on pacifier is very happy. 100% breastfed. Mom
works hard to get baby fed. '

Could it be that this is one of those babies who finishes a feed in a very
short time, and just doesn't want to stay any longer on the breast?  Then,
the mother can't believe the baby is finished, and tries to keep him on,
which leads to the baby crying and squirming, not in pain but to get away!

A few babies at 4 m may take no more than a minute or so on some occasions.


Can't see any reason, from your post,  why it's reflux.
