Anna,

you shared your dilemma with the mother you are supporting:  "I have rarely met
a woman who can be so positive in the face of
overwhelming odds, but I just hope supporting her optimism isn't
unrealistic."

I hope that the organisation you work for can provide you with some supervision
and support on this one.  Sometimes it is from exploring the limits of what we
can provide and promise a woman that we grow the most.  I have always found that
-- aside from insurrance ;-) -- this is the bottom line of what I need from the
organisation I do my Breastfeeding support work for.
