She didn't ask specifically about formula, but she did ask about the
risk of breastfeeding after taking a certain drug. This leaves the door
wide open. I talk about relative risks. Most people know that nothing
in life is without risk. You need to balance the unlikely risk of the
drug being a problem with breastfeeding, against the known risks of
formula feeding. This approach allows that the mother could be informed
about the risks of formula, rather than attacking her decision to
supplement as an ignorant one, which risks putting her on the
defensive. Leave the door open for her to ask "Risks? What risks of
formula?" and then you can go into your spiel.
