Nothing comes to mind immediately other than possible previous
injury to that breast.  However, if I were seeing a mom with such a
concern, I would early on mention to her that "worst-case
scenario"-it is possible to nurse on only one side.  Then we would
go on trying to uncover the cause of the pain and what needs to be
done to correct it.  I find that just knowing that it doesn't have
to be a matter of both or nothing, mom often relaxes and the problem
either goes away or is reduced.  In my 30 years as LLLL including
the last 16 years as LC, I have only known of a couple moms who for
whatever reason started nursing one sided and continued that way
throughout the nursing relationship.  If the cause of the pain can't
be discovered, she may feel better about nursing on the comfortable
side and not continue to be frustrated with the one that is giving
her a problem.  She may want to use a couple extra bra pads on the
"unused" side to look more balanced.
