Hi everyone,

First of all I have to say I´m glad to be back, I´ve
been off lactnet for almost two years - for wonderful
reasons. I´ve started a private practice as an IBCLC
here in Madrid (Spain) and I´ve had my fourth child,
beautiful Isabel. So, busy busy, and oh missing those
lactnet moments!!!

Anyway, I have a question I´d like to share about hep
C. A neonathologist called me to ask if a mom with
cracked nipples should breastfeed or discard until she
is healed.  According to CDC guidelines, she should
discard the milk until there nipples are healed. But
does the viral load play a role in this decision? 

The neonathologist told me that at her hospital,
mothers with an active hepatitis C infection and a
viral load larger than 10.000 are always discouraged
from breastfeeding (regardless of the condition of her
nipples).

Apparently this is based on a 1998 article from
Hepatology journal, which I have not been able to
access, in which there are three case reports of HCV
transmission through breastfeeding (I don´t know about
the condition of the nipples or the mothres´viral
loads). 


Do any of you know of any protocol on handling of HCV
infected breastfeeding mothers?
