I'm an IBCLC at a WIC office in California, USA, posted with permission.

We're working with a mom whose infant has severe allergies, including food allergies. He has been on and off complementary foods since 6 months, with little effect. He's under a doctor and allergists care, but thus far they haven't found foods other than breastmilk that he's able to tolerate.

He came in today, and the doctor had written a note saying he was to receive nothing but breastmilk for at least 3 more months (till he's 15 months). My supervisor (an RD) is worried about the baby not receiving enough nutrients in breast milk without ANY complementary foods. 

I'm a relatively new LC, and very new to this agency, and I'd prefer to have a little more information before proceeding.

My question is this-- what research or information do you have about truly exclusive breastfeeding past 12 months, with no other foods? Is this child liable to suffer vitamin deficiencies, as my supervisor fears?  Is there any data on the makeup of breastmilk and its ability to provide sufficient nutrients for a 1+ year old? My supervisor is leaning toward recommending a breastmilk fortifier that is hypoallergenic, but I'm ambivalent. 

Also, do you have any general recommendations for a baby with such severe allergies?
