I'm only 36... so don't qualify for "over 45", but I thought I'd chime in
anyway.  I learned to hand express through sheer desperation.  I went to
work one night for a long, 12-hour shift and forgot my pump at home.  I was
used to producing roughly 14-16oz of milk in that time frame so I quickly
became desperate to empty my breasts.  Somehow the thought occurred to me
that we milk cow teats, why not human breasts?  (I had no lactation
education behind me at that time and had never even heard of
hand-expression.)  So I just leaned over and began massaging and then trying
to imitate my baby's mouth movements with my hands.  I was shocked and
pleasantly surprised to find that I was able to express the same amount by
hand as I was with my pump!  It was a very empowering experience!
