Luann, have you tried to do a 24 hour total volume intake with this baby?
Low volume may be the answer.  

I had a client yesterday with a 5 month old who hadn't gained any weight for
the last month and was falling off the growth curve.  The baby appeared
healthy in every way and developmentally appropriate.  In my office I
measured intake for one feeding to be 2.4 oz.  This by itself wasn't enough
information, because 5 month old babies are very easily distracted when
eating, and because we know that a baby might eat different volumes of milk
at each feeding.  So I sent the family home with a baby scale to measure
intake for 24 hours.  This baby's 24 intake was only 20 ounces!
