It is quite a process to post a query to JCAHO, but I did so last night. This morning, I received their prompt response. I have posted it in full below because I can't just hit Reply to ask their permission.

ANSWER: 
The following definition of exclusive breastfeeding (based on CDC language) will be used for the measure: 
Exclusive breast milk feeding is defined as a newborn receiving only breast milk and no other liquids or solids except for drops or syrups consisting of vitamins, minerals, or medicines. 
In addition, the notes for abstraction will contain the following statements: 
If the newborn receives any other liquids including water during the entire 
hospitalization, select allowable value �No�. 
Exclusive breast milk feeding includes the newborn receiving breast milk via a bottle or other means beside the breast. 
The response to the question of exclusive breast milk feeding will be based on documentation in the medical record just as it is with all of the other core measure questions.
