Hi all,

Just had to contribute - this might seem obvious or simple (sometimes those are the ones that get us!) but one time a colleague was working with a mom who had a terrible looking yellow crust on her nipples.  This comes to mind because we were going through old clinical photos with a midwifery student and came upon the picture of this mom - I looked at a photo of this mom and said "oh no! Is that a terrible staph infection?"  It was really horrible looking! but it was actually a mom who had very dry, chapped skin on her nipples who was using olive oil - the oil soaked into the dry skin, there may have been some bit of scabbing as well, and it turned the dry skin very yellow, looked crusty too........ when baby came off the breast the nipples were a normal color.  You might want to ask this mom if she is putting anything on her nipples to moisturize or protect them.  I know she is exclusively pumping and you say she is very good at cleaning pump parts but if she is regularly wiping off her nipples or her pump parts are tight and rubbing a bit she might being using a lubricant or cream of some sort to keep things comfy........
