Unless I'm missing something here it sounds like lactogenesis was delayed
and that now she is engorged. You mentioned that her areolas were puffy. I
wonder if her ankles and fingers were swollen, as well. While moms are
edematous the milk is often delayed. I also wonder if she has been spending
a lot of time on the pump when nothing was coming. That can cause more
edema. I'd get her on her back with ice packs all over her breasts as
often as she needs them for comfort and to help reduce edema so she can get
milk flowing.

I was thinking the edema might be an issue too and it's easy to look at
mom's ankles and ask how they are normally. A few handfuls of fresh parsely a day
can really help naturally. I like the lying on back idea to move the fluid,
but I remember that for me personally, the engorgement was so bad I couldnt't
breathe like that, so another possibilty is to her her sidelying with a
rolled up towel to support the top, really heavy breast. Without the towel I had
back pain from the top breast pulling down so much and could neither rest nor
sleep.
Ah, such fond memories of my bad postpartum experience, but I guess they
come in handy sometimes...:)
