<< Most often, baby had an extremely strong suck that exacerbated a problem
probably caused by poor positioning. I would have mom stop breastfeeding for
2-4 days, use the hydrogel dressings, and bottle feed EBM with Av--- nipple.
We had no
trouble getting baby back to breast, in fact, moms reported babies' suck
was gentler after bottling a few days. It seemed that they were forced to
tone down their suck when taking the bottle, or risk drowning. Sorry, got off
the track, and know I'll get some flack for using the
bottles-but it worked. >>

Many times with poor positioning and extremely damaged nipples there was also
poor transfer of milk. These babies are hungry and this is why you see this
"extremely strong suck" or frantic baby. The baby did not learn to suck more
gently from using a bottle nipple, but he is now relaxed and well feed. I'm
sure many of us who follow rule number one (FEED THE BABY) see this. You have
been fortunate to have your babies go back to breast. While nipple confusion
doesn't occur with every baby, it does with some. I prefer to supplement with
SNS with improved latch and positioning (when baby gets milk, you will see
that gentler, suck, swallow pattern) or finger feed a day or two while waiting
for nipples to heal with mom pumping.
