Good evening all!

Couple of questions for you tonite.
I have a client who has a premie born at 1 day before 6mo gestation.  He
weighed in at 1lb 9 oz.  He is currently under bili-lights for jaundice
(don't know the levels) and is under sedation because he gets stressed out,
shakey and screams.  Mom says it's because he's over stimulated by the
bili-lights.  Mom is a 1-2 pack a day smoker, did so thru pregnancy.  Hosp is
feeding baby Nutremegen (sp?) instead of moms breastmilk. My questions are:
1) Could the baby be going through nicotine withdrawl?  I have been told that
an otherwise normal premie. even this young would not have to be sedated as
this one is.  Can nicotine withdrawl in the premie cause him to get shakey
and scream and thrash around?
2) Could the hospital NOT be using moms milk because of the level of nicotine
in the milk?  She said that they are giving the baby Nutremegen because it
was easier for the baby to digest.  (They are gavage feeding, obviously)  I
would think that even this young a baby would be better off with moms milk,
if it was safe for the baby.
