Our hospital routinely screens all admissions with a nasal swab.  Anyone who is found to be positive, regardless of active infection, is placed on contact precautions.  The patient is also flagged within the system and is automatically placed on precautions with every admission until three negative swabs are obtained at least one week apart.  All staff are required to follow the guidelines; however, visitors are instructed on proper hand hygeine and not required to gown and glove.  I would imagine that if a MRSA swab came back positive without an active infection, the mother could pump and feed without regard to a positive swab.
