Hi Amy,

I'm a La Leche League leader in southern MD.  I volunteer on our helpline here. The document process we use is likely similiar to yours (name, contact info, child name, date, mom's comments/concerns, and information given).  That's a great place to start.  Since you are staffing with LCs you might want to include "diagnosis" and/or treatment regimine.

Once you have hammered out exactly what you want/need to document entering it into MSAccess is a relatively simple thing to do.  First off, and MOST importantly, you need to start off with a good database design.  Once you have the design down, setting up the db is a piece of cake.  I would also strongly recommend designing some input forms for the db.  This will make it faster and easier to input information.  It will
also greatly reduce input errors.
