We have recently begun use, in our hospital, of a latch assessment tool
based on the Via Christi model originally created by Jan Riordan. Scores (of
0-2) are given to five different factors in one breastfeeding session --
including mom's evaluation.  The highest possible is 10; the lowest is zero.

Our chart is to be filled in by the nurses (or the IBCLCs; we are there
every other day).  However, we share with mom what we are doing, and why we
have assigned a particular score.  Initially, mom is to be assessed once
every eight-hour shift, until she receives two consecutive scores of 7 or
above.  Then, she may be assessed once every twelve hours.

Our reasons for using the chart: (1) to move away from non-informative
blurbs in the chart (what exactly does "BF well" mean? or "BF poorly?"); (2)
  Identify moms who would need close post-discharge follow-up; and (3)
empower moms with knowledge to assess latch on their own.  Haven't we all
seen that first-time mom who thinks her baby is nursing well ... when babe
is not actually suckling at all?
