I don't profess to know a lot about the needs of the VLBW infant so
certainly won't dare to argue the point with any of our much more learned
friends.  However I do have to points to make.
 Firstly Alan Lucas' work, which he presented here in Brisbane a couple of
years ago.  There was no fortifier used for those babies and their outcome
was still way better than the artificially fed infant.  Interestingly after
presenting all his data he then concluded that breast milk was the way to
go, but add some fortifier to it.  I was pretty stunned, because his data
didn't mention fortifier once before then.  All I could conclude was that
the HMF was going to be better again, no research needed, or that it had
something to do with the formula company that sponsored him :-)
Second point - we do use fortifier where I work.  We have a level 2 nursery,
so they come back to us already on it.  But we only continue to add it to
feeds the baby doesn't get directly from the breast - ie as they get more
and more breastfeeding they get less and less fortifier.  That didn't sound
like the regime the original poster was using.
