Hi Carol,
The Pediatric Clinics of North America, April 2001 Part 2, has a excellent
chapter written by Paula Meier. Paula very clearly addresses this issue and
felt that "nipple shields increased milk transfer by compensating for weak
intraoral suction pressure that would otherwise limit milk intake".  The
chapter is called Breastfeeding in the special care nursery.
