Two additional possibilities:  if this mom had a long birth, received
abundant IV fluids, then she is probably edematous, which means that it
is very difficult to move the milk.  RPS!! Or the other possibility, is
that she has long large nipples, which allows the baby to latch only
onto the nipples and not milk the breast.  She may not even realize
this, especially if she has not seen other women's breasts, or has been
told by everyone in the hospital that she has
 "perfect" nipples.  This is probably more likely the case since the
baby did breastfeed according to her report.  
I agree with Cathy that this mom, as in all cases, has to be seen.    I
manage an internet forum.  When you get such a question, you have to
refer onward.  I would send the site for RPS, for hand expression, and
give her temporary instructions to feed the baby, maintain the milk
supply, and get help to correct the breastfeeding as soon as she can!!
