Hi ladies, 

I have been given permission to post this from a friend that has this
problem.  I gave her feedback about it during our phone conversation (have
not seen it) thinking she described a galactocele, then I archived on
Lactnet about galactocele's and lumps, and sent her via email a description,
and possible ways to proceed,  but I didn't see anything on recurring
galactoceles, doctor not dx properly, doctors' potential concern to it
coming back considering they were surprised it was breastmilk that was
aspirated. 
Her history is below, she was very thorough when I asked her to give me
detailed information before I presented it to you - so it's long.
I have one question - anybody been successful using cabbage leaves to reduce
supply?  Is that a ridiculous question, and why I didn't see it in the
archives? Sorry, though I have been an IBCLC for 3 years, this is my first
galactocele.

Her case:
I am a 47yr old woman and had my first child at 40 and second child at 42.
Both pregnancies were to term and the births normal.  I breastfed my first
child for 24 months and my second child for almost 5yrs.  My children are 26
months apart so I had a brief period (2 mos) of nursing cessation between
children and because I was so close to the birth of my second child, I do
not recall any significant physical changes in my breasts.  I immediately
breastfed my second child and continued to nurse until March 2007.  My
breasts did not seem to undergo any significant changes after nursing
cessation of my second child.  My milk production always seemed appropriate
to the frequency of nursing from high production in the beginning to just
comfort nursing on the right side only,  once or twice a day for the last
year.  I have never had a plugged duct or mastitis during my years nursing.
On 4/27/2007 during a routine visit to my OB/Gyn a "lump" was detected at 9
0'clock in my right breast.  The lump was palatable and about the size of a
large almond.  I was referred to an imaging center for an ultrasound.  The
MD determined that the ultrasound was "inconclusive"  and I was then given a
mammogram.  The MD determined the mammo was also "inconclusive" and FNA
(fine needle aspiration) was ordered.  I had the FNA on 5/24/07 and a that
time approx. 15mL of breast milk was withdrawn from the "lump".  The
procedure was quick and mildly painful.  My right breast did not change in
size or shape however I did notice some pressure relief - similar to the
deflation of a small balloon in my right breast.  The MD cleared the case
and I was released.
On 7/16/2007 I noticed a "lump" about the size of an almond at 9 o'clock in
the right breast - the exact same place as the previous diagnosis.  The lump
is palatable and has a fluidic sensation.  I feel a slight amount of
pressure when I bend over or lie down - similar to an inflated small
balloon.  The pressure is not painful or uncomfortable.  I can still express
milk from the right nipple if I apply pressure.  I have chosen not to
contact my MD until I can gather some additional information about this
situation.  The MD's seem somewhat unclear about my case - they were visibly
surprised when the FNA indicated breast milk!
