This poor lady needs to have a diagnostic ultrasound of the breast. Perhaps
she has/had an abcess that has encapsulated. She most certainly needed more
IV abx than she got. We had a mom with an abcess that was I&D'd and a huge
mess. All because she had a plugged duct, s/s of mastitis and was told by an
ER doc to not breastfeed on that breast. No oral abx at that time. Cultures
of the abcess usually grow staph or strep due to those bugs being commonly
found in noses and mouth (of babies) . If she has scabs on both nipples,
maybe prior damage, there was the portal of entry. Did anyone suggest cabbage
or cold packs on that inflammed breast? How about NSAIDs for the
inflammation? Luckily we were consulted on the above mentioned patient, who
was in a great deal of pain. Surgeon asked what should be given for pain and
I suggested morphine - really. She was writhing in pain, rating it an 11!
