Anne,

Things that jumped in my mind and you may already be trying but just didn't mention: Offering to nurse at baby's earliest cues -- before nap is totally over -- so baby will be most patient and less rough initially when nursing, maybe giving mom more time to trigger wider gape for initial latch; using breast compressions right away while nursing and throughout (will facilitate increase in mom's milk supply AND keep baby happy at breast longer); only giving some formula when baby becomes too fussy at breast and then finishing at the breast as well, when baby's had an ounce or two for formula and is more patient again; reminding her that short frequent nursing (as opposed to longer, more widely spaced) will increase her mothering hormones and potentially affect her mood in a positive way; using her own milk and some air to help nipples heal.
