I think sometimes babies pull back from the breast because they are resisting pressure on the back of their heads or simply trying to adjust and get more comfortable.  Switching to a position in which gravity holds baby to the breast instead of mother's hand can sometimes decrease this pulling or at least make it easier for all to get comfortable.
