(I'm behind again, so it is quite possible these suggestions have
already been given)
I would suggest considering Domperidone (archives have loads of info on
it and how to get it.)  or MoreMilk Plus (don't know how much it's been
discussed on LN-I can send you more info if you don't find it there)  We
have a couple docs that will prescribe Domp.  Moms seem a little
reluctant to order drugs from a foreigh site.  They can get the MM+ at
local health food/nutrition stores.  Off the top of my head, I have seen
at least 80% get a good increase when taking it along with frequent
breast stimulation (nursing, pumping or hand expressing).
The more mom can relax-not always easy when baby is having so many
problems-the better.  Encourage her to use whatever techniques help her
relax.  These might include breathing-like some of the birthing
breathing; visualizing a peaceful scene; quiet relaxing music, etc.  I
also encourage her to have someone give her intrescapular massage.  The
massager stands behind her as she pumps.  Hands are rested on her
shoulders and the person massages her between the shoulder blades with
their thumbs.  I have seen moms go from dribbles to sprays with this.
