I would ask the mom which doctor she wants it sent to.  If there's an NP in
the practice that mothers routinely see, you could ask about sending the
report to the NP, with a note saying you're not clear on which dr the mom
will be using.

If she ends up going somewhere else altogether, well, as you say, you still
have your copy if it should become pertinent.

Might be worth having lunch with the midwife someday, and sharing with her
some of the literature on which you base your approach.  If she's like most,
she'll find the info helpful rather than threatening...
