Ahhhhhh....I get it now :)

The trouble is, British doughnuts do *not* always have a hole in the 
middle of a flattish pastry.

Some of them do, some of them don't.

If I go to my local baker's, the doughnuts he has are round, sugary 
balls with custard or jam in....a bit like an apple, in fact. This is 
what I think of when I think of doughnuts. Maybe it's a regional 
thing, but I don't think so.

We do have the other sort, as well, but the main difference that 
springs to mind is the hole in the middle, not the flatter shape.

In my experience, the majority of women don't actually need to do 
anything with the shape of the breast to achieve a good latch. 
However,  some women are indeed helped by shaping the breast so it is 
flatter, and as long as the nipple is in the top of the mouth so it 
can go right down to the back and not be 'met' by the tongue, then I 
don't care what the mother does, if she is happy and comfortable and 
the baby is too.
