Ilene, a former Peer Counselor had a lump in her armpit--turned out to be a
galactocele.  I don't remember what she did about it--it was 5 years ago.  I
can get in touch with her if I need to.  It's one possibility for your
client, though.  And, yes, any lump should be checked out.
