Subject: any IBCLC's from large (4000+ bitrths/yr) US hospital need rapid feedback

Mary,

I don't have time to specifically address all of your questions as I am at work, however here's an overview of what we do at our hospital with c-sections and breastfeeding. (we delivery about 2700 babies per year)

Our moms are usually in recovery for about 1 hour. We take babies to recovery to breastfeed upon request. Our recovery room accomodates 2 patients in the same room. The baby goes for a brief visit to the nursery along with the mother's banded support person, who requests that the nursery staff hold the bath until after the first breastfeed. The nursery gets a quick set of vitals to determine that the baby is stable. The baby is then released to the support person who is allowed to take him/her to recovery. If a lactation consultant is available at the time we will be called to assist the mom in recovery with breastfeeding. Actually we don't do much but place infant skin-to-skin and prone with the mom, allowing for self-attachment. We reemphasize to the support person that they are responsible for placing infant back in bassinette and returning the baby to the nursery should something happen with the mom during recovery. Most of the time mom is
transported to her room on Mother/Baby still skin-to-skin with her baby.

When we first began doing this we got TONS of resistance from all sides, but is has since become fairly routine.

Please feel free to e-mail me off-list if you have further questions.

Warmly,

Holly McSpadden
