Hi all, and happy holidays!

I´d like to share a case with you, with mother´s permission.

This is the mom of a two month old baby. I have not personally seen her,
since she lives far away, but i´m closely working with her midwife
online/phone.
At one week postpartum, mom started having deep breast pain and nipple pain
in both breasts, during and after feeds. Went to her HCP and she was given
probiotics, which did nothing for her. Two weeks later, she contacted the
midwife who is currently helping her. Same pain, with vasospasm after
feeds. Midwife corrected position and latch, suggested warm compresses
after feeds, but it did not help much.Ten days later, mom´s pain continued
and she had an oversupply (she had started to pump after feeds thinking it
would relieve her, but it did not). Midwife helped her to reduce production
comfortably.

At a month and a half, pain continues and midwife does a home visit (and
she called me from the house to share what she was seeing). What she saw
was that, as baby fed from one side, mother´s other nipple would blanch on
and off, maybe three or four times (painfully, of course). If they
unlatched the baby, that one side would be having on-and-off vasospasms
too! This goes on betwen feeds, just sporadic vasospasm, during and between
feeds.

Midwife kept working on latch, although everything looks good and baby is
gaining beautifully; she ruled out tongue tie; I prescribed nifedipine 30
mg and mother took it for 15 days with no change; baby has gone to CST and
mother has gone to CST, with no change; we´ve cultured the milk and there
is no infection. Pumping is not an option, it does not stop the
vasospasming... We are at a loss about what to try next. Mother is a
fighter, does not want to give up but is crying her heart out and I fear
postpartum depression. We just don´t know what to do about this crazy
vasospasming.

Any ideas before we give up???
