Hello collegues,
I need help from those from India. Frequently, these moms supplement
with formula. This mom may be the exception, but first baby, had fourth
degree laceration. I encouraged skin to skin, showing her feeding cues,
described how to support her breast and baby would latch beautifully.
Her mother came to visit and insisted that she needed a nipple shield
and actually provided her with one. Another time I assisted her in a
side lying position, which was very comfortable for her and baby again
latched beautifully, colostrum was visible in the corners of his mouth.
Her comment was "He knows you". I told her he was ready and hungry. She
and other mothers' of this culture seem to shake there baby to calm them
if they are crying. Even while at the breast. I had asked her if she was
holding a fork to eat and someone shook her would she be able to
continue eating? She agreed that no she would not. I compared that with
baby eating. I talked to her to help her relax her shoulders and her
arms around the baby and she also noticed that he fed very well. She
started to supplement last night and this morning because her milk isn't
in yet. He has not lost very much weight. His stools are changing to
brownish green. She continues to feel that she needs to use the shield.
She wants to wait until the baby cries before offering the breast. She
tends to put him in the crib away from her or send him out with the
nurse. 
I want to be able to teach effectively without breaking any culture.
Your help would be appreciated.
