What a gem you are!  Thank you so, so much.  What 
you've so carefully translated and pasted is 
exactly what I needed, and because the names of 
the authors are in amongst all the Spanish text I 
know exactly which studies have been used as the 
basis for the 2007 recommendations, they are used so often, elsewhere....

I don't know if these are the latest recommendations, I have sent a message
to a colleague in Spain who may be able to help.

For your other questions, the guidelines are very clear (and incidentally,
are the same as those in France).

*7.3.- Recomendaciones
• Las madres con VIH deben abstenerse de forma absoluta de lactar aa sus
hijos y alimentarlos con fórmula adaptada (Nivel A).

"Mothers with HIV must absolutely refrain from breastfeeding their children
and must feed them with appropriate formula."*

The entire chapter on breastfeeding and the risks of vertical transmission
of HIV cites studies "proving a much higher risk of transmission directly
from breastmilk." (pages 60-65)

*En el momento actual, la única estrategia eficaz para eliminar el riesgo de
transmisión a través de la lactancia materna es su absoluta sustitución por
la alimentación con fórmula adaptada. El riesgo de infección por la
ingestión de un litro de leche materna podría ser equiparable al de un
contacto heterosexual no protegido.*

*"At the present time, the only effective strategy to eliminate the risk of
transmission through breastfeeding is its complete substitution by feeding
with appropriate formula. The risk of infection from the ingestion of one
litre of breast milk is comparable to the risk of an unprotected
heterosexual contact.*"

There are no recommendations or mention of pasturising milk to inactivate
HIV.

I've pasted the text of the relevant pages, do let me know if you'd like
more details on the studies cited.
